If you received a sacrament at Christ Our Light Catholic Church, please fill out the following Authorization to Release Form below. Once the sacrament certificate is ready, you will receive an email or phone call so it can be picked up at the Parish Office.

Pick-Up Schedule
- Ensure that you have received an email/call from our parish staff
- Stop by the Parish Offices on Tuesday, Wednesday, or Thursday between 9:15am - 1pm OR 1pm - 4pm

- I hereby authorize Christ Our Light Catholic Church in Navasota, Texas to release a copy of the aforementioned sacramental record requested. I agree to indemnify and hold harmless the Catholic Archives of Christ Our Light Catholic Church of the Archdiocese of Galveston-Houston, the Roman Catholic Church, it's Diocese, Archbishop, Bishops and their successors in office, the aforesaid parish and all other persons with them from any liability for releasing this information pursuant to my request.
